Dental Insurance:
Dr. Colletta’s office accepts numerous dental insurance plans. Please contact our office to confirm that we are currently working with your insurance provider.

Payments:
For your convenience Dr. Colletta’s office accepts MasterCard, Visa, personal checks and cash.

We also offer financing through CareCredit. CareCredit is a third party financing company that offers a credit card designed to help you finance health and wellness expenses – this option is subject to credit approval from CareCredit. Click here for more information.

Office Policies:
Cancellations – If you do need to cancel an appointment we ask that you provide our office with 24 hour notification. Any appointments that are not cancelled within this window may be subject to a charge.

Pre-registration Form:
All first-time patients must fill out a pre-registration form prior to their first appointment. This form can be filled out within our office. If you would like to fill it out before your first appointment please click here to download the form, fill it out, and bring it in with you.
